I agree to my child participating fully in RYC (Riccarton Youth Community) activities. In participating, I agree that it is expected that young people, including my child, will follow the rules and instructions of leaders to ensure the safety and enjoyment of other members. In the event that they can not follow these instructions and restoration can not be made I understand they may be asked to leave the programme. In this situation, I will be contacted and asked to collect them.
I consent to my child being taken out on excursions with RYC as arranged from time to time, using both public and private transport in vehicles with a current WOF, registration, and driven by people with full drivers licenses.
I have supplied all relevant health and dietary information needed to look after my child, and provided details of emergency contacts.
If I cannot be expediently contacted in the event of an emergency, accident, or illness I give my permission to the leaders of RYC to provide and seek medical attention for my child as required. If more than basic first aid is required, I consent to my child receiving such medical, surgical or anaesthetic care which may be needed/determined by the appropriate medical practitioner or hospital authority to which my child has been taken for care. Should an ambulance or other transport be required, I accept responsibility for all expenses incurred.
If tablets or other medications are to be sent on camps or other activities. I will make the leaders aware of this so that they can ensure they are taken correctly.
I accept that this information will be kept on record and will be used for running RYC activities and administration purposes including regular update emails. I expect that all information pertaining to my child shall be kept in accordance with the Privacy Act 2020.
I give my consent that any photographs/videos taken of my child on RYC activities can be used for RYC and Riccarton Community Church (RCC) publicity and administrative purposes, including, but not limited to, use on social media, website and printed material.
